Equipment-related critical incidents in a general intensive care unit.

چکیده
Introduction The complexity of the care provided for critically ill patients frequently leads to medical errors with potential harm. To improve standards of care, each incident should be recorded, reported and discussed within the team. The reporting system highlights where errors were made and what lessons can be learnt from such incidents. These are commonly reported as “critical incidents” or “serious incidents”. A study by Welters et al (2011) found equipment errors accounted for a large number of critical incidents (CI) [1]. It has been found that CIs may lead to longer hospital stays, increasing the cost of patient care; therefore further research into CIs and their prevention is justified [2].

Major sources of critical incidents in intensive care
INTRODUCTION In recent years, critical incident (CI) reporting has increasingly been regarded as part of ongoing quality management. CI databanks also aim to improve health and safety issues for patients as well as staff. The aim of this study was to identify frequent causes of adverse events in critical care with the potential to harm patients, staff or visitors by analysing data from a volunt...

We reviewed all patient safety incidents reported to the UK National Patient Safety Agency between August 2006 and February 2007 from intensive care or high dependency units. Incidents involving equipment were then categorised. A total of 12 084 incidents were submitted from 151 organisations (median (range) 40 (1-634) per organisation). Of these, 1021 incidents were associated with use of equi...

We aimed to determine the type and frequency of critical incidents in a multidisciplinary intensive care unit, to determine outcomes consequent to these incidents and to devise corrective strategies. Prospectively collected data on critical incidents during a 33-month period were analysed. In all, 1918 patients were admitted to the unit during the study period. Each incident was analysed in det...
